Just before a March 2005 explosion at BP's Texas City refinery, workers returned to a temporary trailer after a lunch BP catered to celebrate safe operations. Within minutes, a nearby isomerization unit belched hydrocarbons that exploded.
BP purports to hold safety in the highest regard, yet tragedy and accidents befall the oil giant with a disturbing regularity unseen among its peers. A panel investigating the Texas City disaster found lapses in process safety — BP was good at reducing workplace injuries, but lousy at ensuring the work routines themselves didn't lead to catastrophe."
