Iowa Workplace Accident: Trench collapse trapped plumber in Ankeny - Media Industry Today - News Media Monitoring: "Des Moines, IA-- Rescue teams pulled out a plumber who was trapped in a collapsed trench on Thursday, June 24, 2010. Officials pulled an employee of a local plumbing contractor from trench at 909 Northeast Chambers Parkway in Ankeny, as reported by the Des Moines Register.

It took rescue teams about an hour to pull the Plumbing Co. employee from the trench, which was located on the site of a future housing development. The worker was reportedly conscious and talking when he was pulled from the ground.

Rescue teams discovered him in a half-standing, half-kneeling position in a 5-foot trench with more then 10 inches of dirt on top of him. His co-workers were able to move dirt away so that he could breathe until he was rescued.

Ankeny deputy fire chief Rob Chiappano said, “When we arrived we couldn’t see him, but we could hear him at the bottom of the trench.”"

The Purpose of Protective Clothing | eHow.com: "Masks Provide Protection
A surgical mask is considered protective clothing. It covers both the mouth and nose and should be worn by dentists and dental health care personnel. Masks protect the individual against sprays of body fluids and blood. The mask shields the worker from blood borne pathogens or other infectious microorganisms that the patient may have.
Eye Protection
Protective eye wear shields the face during dental proceeds. The mucous membranes of the eyes are protected from coming into contact with microorganisms."

Unions press penalties for workplace violations | Labor & Employment > Workplace Health & Safety from AllBusiness.com: "Major accidents in the petroleum and mining industries that resulted in 47 deaths demand tougher government regulation and enforcement, union leaders and a government official said Tuesday.

'There is a pattern in recent weeks of fires, major injuries and fatalities in the oil sector, and we're not immune to the fact that the same things are going on in the mining and other industries,' said Leo Gerard, president of the United Steelworkers.
Gerard, United Mine Workers President Cecil Roberts and David Michaels, assistant secretary for occupational safety and health at the Department of Labor, talked to reporters during a first-day break of a two-day emergency meeting of USW oil industry workers. High-profile disasters put new focus on workplace dangers : BusinessJournalism.org Reynolds Center for Business Journalism

High-profile disasters put new focus on workplace dangers : BusinessJournalism.org Reynolds Center for Business Journalism: "We’re not even halfway through 2010 and already have seen some high-profile workplace disasters, including 29 dead in a West Virginia coal mine and the 11 workers lost two months ago when the BP oil rig collapsed in the Gulf of Mexico.

Those epic tragedies make headlines, unlike the many other workplace injuries and even deaths that occur daily nationwide. Some 5,214 U.S. workers died as a result of occupational hazards in 2008, according to federal statistics. That was down from 5,657 a year earlier – in part due to the slower economy, according to Bureau of Labor Statistics analysis – and the lowest since the BLS began recording a fatality census in in 1992."

Proactive Health & Safety Solutions: Bill 168 Is Now Law - Are You Prepared?: "Well the long awaited Bill 168 (now the amendments to Ontario's Occupational Health and Safety Act) can now be enforced by MOL Inspectors. Rest assured that as inspectors now doing their visits to employers will be asking about your Workplace Violence and Harassment Policy and Program.

In order to avoid getting compliance orders you should have the following:
A Workplace Violence and Harassment Policy (one document or combined is ok) posted in a conspicuous place at all you work locations

As Gulf Oil Disaster Worsens, Concern for Safety of Oil-Rig Workers Gets Less Attention

As Gulf Oil Disaster Worsens, Concern for Safety of Oil-Rig Workers Gets Less Attention  "WORKERS AT HUNDREDS OF GULF OIL RIGS FACE DANGERS

Worker advocates are quick to acknowledge the severity of the ongoing leak. Still, many also warn that ignoring the worker safety issues exposed by the tragedy would be a mistake, especially considering that hundreds of oil and gas rigs — among the most statistically dangerous workplaces in the country — remain active off the nation’s shores.

“The worker-safety issue has been completely lost in this story,” said Tom O’Connor, executive director of the National Council for Occupational Safety and Health, an advocacy group. “It’s one of the biggest industrial disasters in recent history, and yet Congress [views it] the same as the public: They’re not seeing it as a worker-safety issue.”"

Workplace Accident Investigations Comply with Employment Law and Use Root Cause Analysis

Workplace Accident Investigations Comply with Employment Law and Use Root Cause Analysis: "When a serious workplace accident occurs, it's imperative that employers retain an independent firm to conduct a prompt and thorough workplace investigation. An incomplete, inaccurate or biased investigation can aggravate the problem and increase your potential liability.

You should select an investigation firm that not only understands OSHA issues and employment law, but also knows how to properly conduct the workplace investigation and perform interviews. Our Investigators have years of experience in conducting various types of workplace accident investigations.

We are also able to perform Root Cause Analysis and review safety practices that can help prevent accidents from occurring."

gas 4 less Corporate Manslaughter Concentrates the Mind of the CEO

gas 4 less Corporate Manslaughter Concentrates the Mind of the CEO: "Accident Investigation is responsible, effective and helps the bottom line.-

'On the continent,' says Lorna Ramsay, Director of the TOP-SET® Investigation System, referring to Europe, there's corporate manslaughter. The CEO can be put in jail if people are injured or killed on-the-job. This concentrates the mind of the CEO wonderfully,' she added.

This statement got my attention. How about you?

I'm interested in the use of 'gut feeling' or intuition in the workplace, among other skills considered 'soft,' and I'd heard Lorna taught engineers who work in nuclear plants, the oil and gas, explosives and other high hazard industries, how to stay safe by using their intuition - as part of using the TOP-SET® Investigation System.

Perfect, I thought, high IQ employees using their EQ to keep alive. It's for sure in today's rapidly-changing workplace, companies all over the world are finding intellect alone is not enough. TOP-SET® teaches that indeed it isn't.

Official Warns Grim Work Safety Conditions in China

Official Warns Grim Work Safety Conditions in China: "China's work safety conditions remained grim as the number of work safety accidents remained large, illegal production still posed challenges and safety management was still loose, a senior official said on Sunday.

Yang Yuanyuan, deputy chief of the State Administration of Work Safety, made the remarks at a forum on work safety in Beijing.

In the first five months this year, 499 people were dead or missing in 36 major workplace accidents such as coal mine flood and gas explosion, a rise of nearly 40 percent from the same period a year ago, Yang said.

Notably, five severe accidents, each with a death toll of more than 30, had happened so far this year, killing 181 lives, up nearly 70 percent year on year, he said.

Yang noted the grave picture reflected poor enforcement of safety rules, and enterprises' mere pursuit of output in sacrifice of work safety.

He said the government would make continuous efforts to bring those accidents under control.

Fatal plunge by stagehand stirs questions of safety, fines - News - ReviewJournal.com

Fatal plunge by stagehand stirs questions of safety, fines - News - ReviewJournal.com: "Las Vegas stagehand Vicente Rodriguez fell to his death from a rigging plank suspended over the MGM Grand's Hollywood Theater. If the planking had been equipped with guardrails, the 20-year-old -- who was incorrectly wearing his safety harness -- might be alive today.

The stagehand's death and the relatively small fines assessed for failing to maintain workplace safety -- imposed according to a penalty scale that has increased only once in 40 years -- are part of a national discussion about improving safety in the entertainment industry.
