An accident is defined as “an unplanned, unwanted but controllable event which disrupts activity and may cause injury to people or property damage.”

Meanwhile, incident is “an unplanned or unwanted event which disrupts the work process but can only potentially cause injury, harm, or damage to persons or property.” It may be a series of events which could lead to harm or damage.

In most scenarios, accident is used to describe both situations, and that includes in the workplace. Workplace accidents have to be investigated by a qualified person who has years of experience and full knowledge of the process."

Iowa Workplace Accident: Trench collapse trapped plumber in Ankeny - Media Industry Today - News Media Monitoring: "Des Moines, IA-- Rescue teams pulled out a plumber who was trapped in a collapsed trench on Thursday, June 24, 2010. Officials pulled an employee of a local plumbing contractor from trench at 909 Northeast Chambers Parkway in Ankeny, as reported by the Des Moines Register.

It took rescue teams about an hour to pull the Plumbing Co. employee from the trench, which was located on the site of a future housing development. The worker was reportedly conscious and talking when he was pulled from the ground.

Rescue teams discovered him in a half-standing, half-kneeling position in a 5-foot trench with more then 10 inches of dirt on top of him. His co-workers were able to move dirt away so that he could breathe until he was rescued.

Ankeny deputy fire chief Rob Chiappano said, “When we arrived we couldn’t see him, but we could hear him at the bottom of the trench.”"

Business Report - Home - Three die in explosion at Harmony Phakisa mine: "Three miners died in an underground explosion accident at Harmony Gold's Phakisa mine in the Free State.

In a statement on Friday, the company said the accident occurred on Thursday night about 2013 metres below the surface.

Harmony added that two other employees were seriously injured and were being treated at the local hospital.

'The employees were part of a Mine Rescue Team that was busy investigating a suspected fire in a raise, underground at the time of the accident."

Gulf Cleanup Training Ignores Advice From Health Agency, Official Says - ProPublica: "As we've reported, workplace safety experts have expressed concern that Gulf oil spill responders aren't getting enough safety training. On Wednesday, we spoke with a federal official who said the four-hour safety course that BP is providing to Gulf cleanup workers lacks basic information on health risks and is too short to cover the necessary material.

Joseph Hughes, director of the worker training program at the National Institute for Environmental Health Sciences, said the course fails to incorporate important information. Among the subjects not included are chemical inhalation, the health effects of dispersants, and the risks of direct contact with weathered crude oil."

BBC News - East Ayrshire council fined over electrician's death: "A council has been fined 56,000 over the death of an electrician who was killed when a work platform collapsed in a school gym hall.

Robert McGill suffered severe brain injuries after the accident at Kilmarnock Academy last April.

East Ayrshire Council admitted a charge under health and safety legislation.

Kilmarnock Sheriff Court heard that the council has since amended its working and training procedures.

Sheriff Seth Ireland said the level of fine was not to be taken as a value on the life of Mr McGill.

He added: 'The loss of Mr McGill to his family must be regarded as being beyond any monetary figure and be priceless.'"

This issue has most recently been brought to the public's attention in the most tragic way possible, with deaths of 11 workers and injuries to 17 others as the result of the April 20th explosion on the Deepwater Horizon offshore oil drilling platform. The Deepwater Horizon disaster occurred even as OSHA continues to deal with the ramifications of the 2005 fire and explosion at BP's Texas City refinery that killed 15 workers and injured more than 170 others, and to help our Washington State Plan partners investigate the April explosion at a Tesoro refinery that left seven more workers dead."

Worker advocates are quick to acknowledge the severity of the ongoing leak. Still, many also warn that ignoring the worker safety issues exposed by the tragedy would be a mistake, especially considering that hundreds of oil and gas rigs — among the most statistically dangerous workplaces in the country — remain active off the nation’s shores.

“The worker-safety issue has been completely lost in this story,” said Tom O’Connor, executive director of the National Council for Occupational Safety and Health, an advocacy group. “It’s one of the biggest industrial disasters in recent history, and yet Congress [views it] the same as the public: They’re not seeing it as a worker-safety issue.”"

Safety Non-compliance on Patato Farm | Health and Safety: "The team, which included the Mangaung municipality’s Environmental Health unit and the Human Rights Commission, issued prohibition notices on the use of all sorting and packaging machinery at the farm.

The farmer, who employs about 50 workers, was issued with various notices of the contravention of the Occupational Health and Safety Act. These included notices for not providing proper personal protective equipment to all workers and not having a valid certificate of compliance for electrical installations.

The notices were valid until the farmer conducted proper induction training for workers in using the machines.

All the dangerous machines were safeguarded as per the Occupational Health and Safety Act.
